What are the Terms & Conditions applicable to a
Make Up Day? There shall be
no work performed without the consent of the Union.
The Steward or Foreman must call the hall or Agent
for permission.
Make up is defined for weather only and in the week
of the lost only. Can not be saved up.
If you worked more than two (2) hours
that day, he can not be used for a make up day.
Once a job goes to overtime, there will
be no make up time allowed.
Employees can not be transferred from
other jobs until all employees on the job have been
asked to work the make up day first.
Make up day applies to Saturday only
Make up day is not mandatory.
2. How do I add a newborn or a new
spouse to my insurance?
For a newborn we need to receive a copy
of the birth certificate showing the parents names.
For a spouse we need to receive a copy
of the marriage certificate along with the spouse’s
date of birth and social security number.

3. What should I do when I receive
a Cobra notice?
If you know you’ve been working,
call the office. This may mean that we have not received
any work hours and your bank has been depleted.
4. When will I be eligible for benefits?
Once you work 500 hours you will be
eligible on the first day of the second month after
you have worked.
5. How can I remove my spouse
from the insurance?
We need to receive a copy of the “FINAL
DIVORCE DECREE” before we can remove a spouse
from the insurance.
6. I lost or misplaced my insurance
card, how can I have it replaced?
Just call our office and we can order
the card.
7. How can I find a dentist who is
in network?

and they can help you find a dentist in your area.
8. How do I find an in network Eye
Doctor?
Call Opti-Choice at 1-800-541-2039.
They can also tell you when you are eligible for your
next visit.
